Report factual errors, outdated hospital details, missing or misrepresented evidence, relationship disclosures, or errors in applying the published methodology. Hospitals may submit evidence but cannot directly change scores.
可提交事实错误、过时的医院资料、遗漏或误述的证据、商业关系披露，以及方法执行错误。医院可以补充证据，但不能直接改分。
Include the page URL, the disputed statement, the proposed correction and dated sources. Please omit patient records and other sensitive personal information.
